Provider Information Collection Documents

Social care providers are required to provide CSSIW with information on their service on a regular basis (for most services this is annually). CSSIW will alert providers by letter when they are required to complete these documents. Reference versions of these documents are available here. We encourage providers to complete and return these documents electronically. Contact details for electronic return are provided with the letter requesting this information.

These documents are part of our work to modernise our services and to make our documentation more user friendly. The aim of these significantly shorter documents is to reduce the burden on service providers and gather a consistent set of information from all services.

When required to complete these documents you will be sent a pack containing the following:

• The basic information about your service form is a simple table showing the basic information that CSSIW currently holds about your service. You are asked to correct or update that information as needed.
• The service data form asks you to provide data about your service. It consists of a series of tables in which to record numerical and factual information about your service users (i.e. the adults or children who use your service) and care staff.
• The self assessment of service statement asks you to explain how you ensure that the adults or children who use your service benefit from a good quality, safe service, bringing positive outcomes both for service users and for the people who care about them.

These documents have a standard format and content across all service types but differ in the detail.
